The $149 is a flat software-usage fee for the MyUSVisa case-management system — the workspace you and your client use to manage the matter once you've agreed to work together. It becomes due when the matter is activated in the system, and it is not a fee for a referral, introduction, or lead, and not a split or percentage of your legal fees. You form the engagement with your client, set and keep 100% of your own fees, and whether you absorb the $149 or pass it on is your choice. MyUSVisa is a technology platform, not a law firm, and takes no part of any attorney–client fee. Attorneys remain responsible for compliance with their jurisdiction's rules of professional conduct.
